Kouba Dermatology is an independent dermatology practice founded in 2026 by David J. Kouba, MD, PhD, who had practiced with the Toledo Clinic since 2008. It operates seven offices across northwest Ohio and southeast Michigan, with its main office at 7301 Secor Road in Lambertville, Michigan and a dedicated Mohs and Superficial Radiation Therapy center in Wauseon, Ohio. The practice concentrates on the prevention, detection and treatment of skin cancer, offering Mohs micrographic surgery with same-day facial plastic reconstruction performed by Dr. Kouba, and Superficial Radiation Therapy as a less invasive alternative, with two physician assistants dedicated to SRT patients. General dermatology is provided to patients aged eighteen and older. The cosmetic line includes botulinum toxin, hyaluronic acid cheek and lip fillers, chemical peels, microneedling and Morpheus8 radiofrequency microneedling, laser hair removal, pulsed dye laser treatment and PRP for hair restoration. The practice also runs a Mohs surgery fellowship.
