Rhinoplasty: making the decision

The thing most people want to hear first: Most revisions are minor refinements — choosing a surgeon who does noses week in, week out is the single biggest factor in getting it right the first time.

Open, closed, and “liquid” rhinoplasty, compared

Three very different things get called a nose job. The surgical versions differ mainly in where the incisions are; the injectable version is a different product category with different physics, different economics, and an FDA recommendation worth reading before anyone touches your nose with a needle.

Worth knowing. The FDA explicitly recommends against injecting dermal filler into the nose — its guidance names the nose, glabella, forehead, and the area around the eyes as sites where these uses are not approved, because vessels there connect toward the eye and an accidental intravascular injection can cause skin necrosis, vision loss, or stroke. Injectors do treat the nose off-label; if yours proposes it, they should volunteer that fact, explain their emergency protocol, and stock the reversal enzyme.

How much does a nose job cost?

ASPS reports an average surgeon's fee of $7,637 for rhinoplasty, which is the surgeon alone — anesthesia and the facility are extra, and the all-in figure is typically $8,000–$15,000. Revision rhinoplasty usually costs more than a first operation, not less: scar tissue makes it harder, and it often needs cartilage grafted from the septum, ear or rib. Ask specifically what the practice's revision policy is before you book, because a surgeon who charges only the facility fee for a revision within a set window is offering something materially different from one who charges full price.

Does insurance cover rhinoplasty?

Not for appearance. It can cover the functional part — a septoplasty to straighten a deviated septum, or turbinate reduction — when there's documented breathing obstruction. The two are frequently done in the same operation, in which case the functional portion may be billed to insurance and the cosmetic portion to you. Get that split in writing beforehand, itemised, because 'insurance will cover part of it' means very different things depending on which part.

How long until a nose job looks final?

Longer than almost any other cosmetic operation. The splint comes off in about a week and most people return to work then, but rhinoplasty swelling resolves in stages over roughly a year — and up to 18 months for thicker skin or after a revision. The tip is always last to settle. Surgeons generally won't discuss a revision until at least a year has passed, because the nose you have at three months is not the result.
