About FoRM Health

FoRM Health (Functional Regenerative Medicine) is an integrative clinic in Portland's Sellwood neighborhood serving the greater Portland metro area. Co-founded by Dr. Ryan Minarik, ND, LAc and Dr. Kai Herman, ND, the clinic grew out of Elixia Wellness Group, founded by Minarik in 2008. FoRM operates three specialty divisions: Orthopedics/Sports Medicine, Functional Medicine, and Aesthetics, with a roster of 16 clinicians.
