About Lori G. Polacek

Dr. Lori G. Polacek is a board-certified plastic surgeon who founded her Cranston, Rhode Island practice in 1991, at which time she was the first female plastic surgeon in Rhode Island. After close to 30 years of surgical practice she transitioned the practice to focus exclusively on advanced non-surgical procedures, and in 2026 relaunched it as Polacek Aesthetics & Wellness in partnership with Formula Wellness. She earned an A.B. at Brown University in 1980 and her MD at Boston University in 1984, and was certified by the American Board of Plastic Surgery in 1994. She has held an appointment at The Miriam Hospital since 1991. Her current practice covers BOTOX Cosmetic, the Juvederm collection of fillers, CoolSculpting, Fraxel Dual and Clear + Brilliant laser treatments, medical-grade chemical peels and Vivace RF microneedling.
