Polacek Aesthetics & Wellness, formerly the Polacek Center for Plastic Surgery, is an exclusively non-surgical aesthetic practice at Chapel View in Cranston, Rhode Island, serving the greater Providence area. It was founded in 1991 by Lori G. Polacek, MD, the first female plastic surgeon in Rhode Island, who after roughly three decades of surgical practice shifted the practice's focus entirely to non-surgical treatment. In 2026 the practice announced a partnership with Formula Wellness and adopted its current name, reflecting an expanded focus on comprehensive non-surgical and longevity-based care. Its menu covers BOTOX Cosmetic, the Juvederm collection of fillers, CoolSculpting, Fraxel Dual and Clear + Brilliant laser treatments, medical-grade chemical peels and Vivace RF microneedling. The clinical team includes Dr. Polacek, a family nurse practitioner, a physician assistant and licensed medical aestheticians.
