Sano Longevity is a concierge longevity medicine practice in the Middletown area of Louisville, Kentucky. It was founded in March 2025 by Meredith Sweeney, MD, FACS, a board-certified surgeon who spent more than a decade as a bariatric surgeon at Norton Healthcare before opening the practice. The practice is membership-based rather than insurance-billed, and operates three twelve-month programs: Longevity Optimization, Weight Loss, and Menopause 3.0. Every member begins with an in-office fasted assessment covering VO2 max and resting metabolic rate on the PNOE platform, InBody 580 body composition analysis, and a 46-marker blood panel run through Quest Diagnostics, followed roughly a month later by a 60-to-90-minute roadmap appointment. The practice serves the Louisville metro and surrounding Kentucky and Southern Indiana communities, with follow-up care available by telehealth.
