Nest Medical Center is a primary care and functional and longevity medicine clinic in Miramar, Florida. It was founded by a double board-certified family and obesity medicine physician and by Rafael Hernandez Lora, MD, an internist specializing in geriatric medicine; both previously held leadership roles at Chen Medical. The practice combines conventional primary care, geriatric care, in-house laboratory work and vaccines with functional medicine, hormone evaluation and bioidentical hormone therapy, testosterone therapy, medically supervised weight loss and metabolic health programs, gut health programs, injectable vitamins, IV therapy and aesthetic treatments including Botox and fillers. Care is structured around a Foundational Health Mapping diagnostic step followed by four- to six-month metabolic, gut or hormonal programs, alongside three monthly membership tiers. The clinic accepts most major insurance plans for primary care visits, offers telehealth, and sells supplements.
