Jowls

Sagging along the jawline — where camouflage ends and repositioning begins.

Facelift / lower faceliftJawline fillerSkin-tightening devicesThread lift

Jowls form when aging takes volume and elasticity from the face and the remaining tissue descends past the jawline. The treatment menu here spans an order of magnitude in cost and permanence, and the honest way to read it is by mechanism: one option repositions the sagged tissue, the others camouflage, tighten modestly, or hold temporarily. None of the non-surgical options removes loose skin.

Why it happens

The drivers are loss of facial volume and skin elasticity with age — and anything that deflates facial fat accelerates it. ASPS has documented the pattern with rapid weight loss, including on GLP-1 medications (“Ozempic face”): the fat that kept skin taut goes, sagging and jowling arrive early, and what's often needed afterward is surgical lifting or fat grafting rather than more deflation.

The one-line version of this whole page: filler defines, devices tighten modestly, threads hold briefly, and only a facelift repositions. Matching your expectation to that mechanism — before you spend — is most of the decision.

Treatment options, honestly compared

The definitive fix — the only option that repositions sagged tissue.

A facelift lifts the deeper tissue layers and repositions them, removing excess sagging according to its severity. It's the structural answer to a structural problem, which is why every other option on this page is described relative to it.

Jawline filler

Non-surgical

Camouflage: sharpens the jawline, doesn't lift the sag.

Juvéderm Volux XC is FDA-approved specifically for improving jawline definition in adults with moderate-to-severe loss of it — the first filler with that indication. Definition is the whole indication: nothing in the approval covers lifting lax skin, and chasing a surgical result by overfilling the jawline and pre-jowl area is how this goes wrong. Well used, it blurs the shadow of early jowls for about a year.

Modest tightening over months — the AAD's word, not ours.

Energy devices stimulate collagen. The American Academy of Dermatology's own patient guidance says most people see modest lifting and tightening within two to six months of an ultrasound treatment, and radiofrequency results appear over about six months and can last two to three years. “Modest” is the honest calibration against a facelift.

Thread lift

Non-surgical

A subtle, temporary hold for early jowls.

Temporary sutures produce what ASPS describes as a subtle but visible lift — positioned for early jowls, not established sagging. The threads dissolve, the result is measured in a year or two, and an unsatisfying outcome is something you wait out.

What it costs

ASPS average surgeon/physician fees — surgeon only, excluding anesthesia, facility, and related expenses. ASPS publishes no fee for thread lifts, so we don't quote one.

Losing weight can make jowls worse

Facial fat is structural: rapid or massive weight loss — GLP-1 medications included — deflates it, and the skin it supported sags. If you're mid-weight-loss and unhappy with your lower face, the sequence matters: stabilize first, then assess, because ASPS notes what's often needed after major loss is lifting or fat grafting, and treating too early means treating a moving target.
