Transform Wellness Clinic is a concierge integrative and functional medicine practice in Los Gatos, California, founded and led by Bradford Perkins, MD, a family physician with more than 20 years in practice. Care is organised into four programs: longevity medicine built on advanced diagnostics, IV NAD+ and peptides; concierge primary care with 24/7 physician access; weight management using GLP-1 therapy and metabolic testing; and a cognitive program combining TMS and peptides. Diagnostics include metabolic and biomarker panels, genetic and epigenetic testing, and body composition and metabolic performance assessment. Membership is $6,000 a year and the practice also bills most PPO plans and Medicare, with laboratory work provided at cost. It serves patients from Los Gatos, Saratoga, San Jose and the wider Bay Area.
