Kulak Eye & Cosmetic Surgery is the solo practice of Dr. Amy Kulak, MD, a board-certified oculofacial plastic surgeon in Jacksonville Beach, Florida, and a diplomate of the American Board of Ophthalmology and fellow of the American Academy of Cosmetic Surgeons. She earned her undergraduate degree at Brown University and her MD at the University of Miami Miller School of Medicine, completed an ophthalmology residency at SUNY Downstate Medical Center, and a fellowship in oculofacial plastic surgery at Beaumont Medical Center and the Kresge Eye Institute. The practice combines cosmetic eyelid and facial procedures with general ophthalmology and dry-eye care.
