The practice was founded in 1988 by Lawrence E. Foltz, D.O., FAAFP, and his wife Erma, a registered nurse. Born and raised in Portland, Oregon, Dr. Foltz returned to the Northwest after completing his medical training and became one of the first osteopathic physicians to practice in Vancouver, Washington. They established Cascade Family Medicine to deliver thoughtful, individualized care rooted in osteopathic principles. In 2008, Dr. Foltz’s youngest daughter, Sherilyn Foltz-Cook, D.O., MSCP, joined the practice, continuing the family legacy of comprehensive, patient-focused care.
